Personalised Support Plan step by step guide
Personalised Support Plan
A step-by-step guide for tutors: from prioritising learners to milestone reviews.
1. Use the dashboard and learner table to prioritise work
- Open the app to see stage cards showing where everyone is.
- The dashboard highlights the learners that need attention first.
- The learner table uses the same language and stage structure.
- Filter by stage to build a worklist quickly.
- Choose which columns to display.
- Once configured, the table remembers your setup for next time.
2. Open a learner record to see the full journey and next step
- When you open a learner, the full journey is visible in one place.
- The system shows the recommended next step.
- This keeps the tutor focused on action rather than searching for context.
- The journey is designed to make the next decision clear and consistent.
3. Review learner evidence and barriers before deciding on support
4. Make the stage-gate decision to proceed or stop
- After reviewing the evidence, make one recorded decision at the stage gate.
- Decide whether to take the learner forward to a needs assessment: yes or no.
- Capture the reasoning either way.
- If you say yes, the plan is drafted and your supporting evidence can be added.
5. Draft reasonable adjustments and define success criteria
6. Conduct the needs assessment with the learner
- In the needs assessment, you walk through only the stages you have chosen to show.
- Confirm the learner information is correct.
- Review the cognitive results with the learner.
- Discuss known barriers and update them if needed.
Co-create the plan by:
- Reviewing reasonable adjustments
- Considering what has worked before
- Adding anything else needed to meet learner expectations
- Rewrite goals together so the learner voice is reflected throughout.
7. Record the learner’s decision and set up support
8. Share, download and evidence the support plan
- Once complete, share the needs assessment with the learner.
- Download a copy for your records.
- Begin recording all support provided on a regular basis.
Log both:
- Standard support
- Above-and-beyond support
9. Capture signatures and maintain compliance
- Above-and-beyond support now includes e-signature capability.
The learner can sign by:
- Scanning a QR code
- Using a link sent by email
- This helps ensure evidence is captured in a compliant way.
- E-signature is also available in the support plan after the assessment meeting.
- Track milestone reviews throughout the learner journey.
At each meeting, whether every four weeks or every quarter, record:
Use the toolkit section to manage:
- Reasonable adjustments
- Learning modules assigned to the learner
- The toolkit is a living area that updates based on learner progress and review outcomes.
11. Outcome: less admin, more learner focus
- The overall goal is to reduce guesswork for tutors.
- The system gives tutors more time to focus on the learner experience.
- Cognassist creates a consistent, trackable workflow from assessment through review.
